PROVIDER AGREEMENTS
All Providers sign a StaffMD Provider Agreement which sets forth all of the basic terms of any locum tenens assignment. Then for each specific locum tenens assignment, the Provider and StaffMD will complete and sign an addendum or supplemental schedule that outlines the specifics of that particular assignment. This addendum will list the applicable pay rates, the location of the assignment, any travel arrangements or special expense items, any special requirements for performance of the assignment, etc. Additionally, the addendum will set forth how each item is being addressed, if applicable. Once completed, the addendum is signed-off by the Provider and StaffMD.
PROVIDER PAY
StaffMD realizes that each Provider is, in effect, trusting us with their livelihood for each worked assignment. Therefore, we want each Provider to clearly understand when they will be paid. Once a Provider begins working on an assignment, it is important that the Provider provide StaffMD with weekly timesheets that have been signed by the designated Client personnel. Each Provider should immediately upon arriving at an assignment, ascertain who is authorized to sign-off on a timesheet for the Client. This signed timesheet is necessary so that StaffMD can pay the Provider for his/her service on a timely basis. Payment can be made via check or direct deposit if the provider provides the necessary bank deposit information.
